Since its inception in 2004, The Horizon Foundation for New Jersey (The Foundation) has committed to supporting organizations that make New Jersey healthier. For twenty years, The Foundation has proudly invested more than $74M in grants to hundreds of organizations throughout New Jersey.

The Horizon Healthy Communities Grant Program was developed to provide vital support to community-driven initiatives to improve health, promote wellness, and enhance New Jerseyans’ quality of life. The Foundation is pleased to collaborate with the New Jersey Center for Nonprofits to shape this program.

To be clear, the program is designed to support small, start-up, and/or newly established nonprofits (annual revenue less than $250,000) that have a health focus, and are not a prior grantee of The Foundation. Grants will be awarded for general operations, capacity building, and/or program support.

Applications will open December 15, 2023, and close March 1, 2024. The Foundation Board will approve $100,000 in total grant awards by April 16, 2024. Grant programs to begin July 1, 2024.
